Support Coordination is a services funded under the NDIS that helps participants understand and implement their plan. It is designed to build your ability to navigate the system, make informed decisions and coordinate your other supports to help you work towards the goals set out in your plan.

A Support Coordinator does;

• Help you understand your NDIS plan
• Connect you with local service providers that meet your needs
• Build your confidence and ability to manage your supports
• Assist you with preparing for plan reviews or changes
• Help solve issues with service providers or funding

A Support Coordinator does not;

• Provide personal care or direct support
• Manage your funding or budgets
• Make decisions on your behalf - our job is to help you to make your own informed decisions
